About the Scholarship

CHED has opened up its doors to provide a fully-funded scholarship to deserving faculty members and non-teaching staff of different local HEIs during the K to 12 Transition Program Phase (SY 2016-2017 to SY 2019-2020). This is in line with the goal of the government to strengthen the qualifications of employees while there is a low demand for their services during the transition stage.

Requirements (All must be original unless indicated as photocopy.)
  • Duly accomplished Application Form (A1)
  • Recommendation Form (A2)
  • Certification from School Head (A3)
  • Rehiring Agreement Form (A4), if not permanent in Sending Higher Education Institution (SHEI)
  • Transcript of Records, for every course taken: BA/BS, MA/MS, units earned (Photocopy)
  • Supporting Documents and Certifications
    • List of courses offered by SHEI
    • Total college/university enrollment (for 2 recent terms)
    • Teaching Load, for 2 recent terms (Photocopy)
    • Certification that grantee has no pending administrative charges
    • Certification that grantee has no pending scholarship programs being undertaken
    • Certificate of Employment indicating the following:
      • Official Designation
      • Status of Employment (Full-Time/Part-Time)
      • Tenure (Permanent, Probationary, Contractual or Temporary)
      • Date hired
  • Medical Certificate
    • Doctor’s Certificate (to be administered by a Government physician)
    • Urinalysis (within 1-month validity)
    • Fecalysis (within 1-month validity)
    • Complete Blood Count (within 1-month validity)
    • X-ray result (within 6 months validity)
  • NBI (within 1-year validity) or Police Clearance (within 1-month validity)
  • Birth Certificate
  • Notice of Admission or equivalent (Photocopy)
  • Approved Plan of Study
  • Program Description/ Curriculum (Photocopy)
