Qualifications:

1. A natural-born Filipino Citizen
2. Must be a Valenzuela City resident (have resided for at least 4 years prior to applying to this scholarship)
3. Must be a Grade 12 Student taking GAS, HUMSS, STEM, and ABM
Requirements:

5. Two (2) pcs. Passport size in blue background with written name and signature (Last Name, First Name, Middle Name)\
6. Certified True Copy of Grade 11 Report Card (1st & 2nd Semester with GWA of 85% & above)
7. Certified True Copy of Grade 12 Report Card of the recent grading period
8. Certificate of Residency and Indigency from Barangay of both parents
9. Employed Parents:
Certified true copy of latest Income Tax Return (ITR 2316)
Unemployed Parents:
Affidavit of Non-filing of ITR
Certification of Non-filing from BIR
Parents working abroad:
Employment Contract of OFWs
10. Actual street and residence view picture. And residence sketch with landmark
11. NSO/PSA Certified Birth Certificate (photocopy)
12. Certificate of Good Moral Character from the school authorities (photocopy)
13. Medical Certificate and Chest X-ray and physical exam result (photocopy)
14. School Identification (ID) card of applicant and Voter’s Certificate of both parents
