5. Income Tax Return of Parents of BIR Certification of No Taxable income (latest, with attached receipt). Parent’s combined annual income must not exceed P 96, 000. 00)
6. Birth Certificate (latest from PSA, with attached receipt)
7. Baptismal Certificate [must be a Roman Catholic]
8. Recommendation letter from Parish Priest (latest)
9. Barangay Clearance of the Applicant (latest)
10. Family Picture [latest, with name caption in paragraph form]
11. Applicant’s house picture [with brief description]
12. 2 pcs. 2 x 2 picture (latest, colored)
13. Autobiography (at least 3 pages, short-sized bond paper)
