Photocopy of High School report card duly signed or authenticated by the Principal or Registrar.
Certificate of Good Moral Character
Birth Certificate (Photocopy)
Two (2) recent and identical 1" x 1"; pictures signed at the back by the applicant.
Official Receipt of Application Fee (P 20.00) and CAT Fee (P 125.00) to be paid by the applicant to the University Collecting Officer. All fees are non-refundable.
One (1) Long Folder (white)

Transferee
