Freshmen
- Go to the Office of Admissions and Scholarships (OAS)
- Fill-out Enrollment Application Form
- Submit requirements
- Get Student ID Number
- Proceed to the Accounting Department and pay P200 for the testing fee and P50 for the digital picture
- Proceed to the OAS
- Present the receipt of payment for picture taking and get the schedule of Entrance Exam
- Take the Entrance Exam at the Testing Room/Guidance Office upon presentation of the Testing Permit to the Guidance Counselor and wait for the test result interpretation, interview and issuance of the duly signed enrollment admission slip
- For Nursing applicants, go to the College of Nursing for physical examination
- Go to the Enrollment Technical Office (ETO) and present the enrollment admission slip for the following: encoding of tentative subject load, printing and releasing of the duly-signed enrollment form, procurement of the Random Drug Testing Notification Form
- Proceed to the Accounting Department for payment. (Students who will pay in full are entitled to a cash discount which will be evaluated by th Accounting Department, brothers and sisters are entitled to a tuition fee discount, payment must be made within 3 working days for your subjects to be officially enrolled.) NO DISCOUNTS DURING SUMMER TERM
- Proceed to the ETO
- Submit the remaining copies of the enrollment form together with the acknowledgement receipt of the notification on random drug testing signed by the parent/guardian
- Claim the Official Study Load, Student Handbook, and ID Card. Check immediately if the study contains all the subjects you intend to enroll and the corect schedule of classes. Changing (adding/dropping) of subjects is discouraged.
Transferees
- Go to the Office of Admissions and Scholarships (OAS)
- Fill-out Enrollment Application Form
- Submit requirements
- Get Student ID Number
- Proceed to the Accounting Department and pay P300 for the testing fee and P50 for the digital picture
- Proceed to the OAS
- Present the receipt of payment for picture taking and get the schedule of Entrance Exam
- Take the Entrance Exam at the Testing Room/Guidance Office upon presentation of the Testing Permit to the Guidance Counselor and wait for the test result interpretation, interview and issuance of the duly signed enrollment admission slip
- Go to the University Clinic for medical/dental evaluation
- Go to the Student Affairs Office (SAO)
- Fill-out necessary form
- Give the countersigned Testing Permit to the SAO Head for interview
- Get Entrance Test Result reviewed and signed by the SAO Head, Enrollment Admission Slip
- Go the College/Department where the course you wish to enroll belongs.
- See the Dean/Chair for the submission of the Entrance Test Result and approval of the Enrollment Admission
- Courses taken from other schools must be initially evaluated bu the Dean/Chair using the prospectus/checklist as basis of you tentative course load
- Present tentative course load to the enrollment adviser for encoding and printing/releasing of the duly signed enrollment form
- Attach the prospectus used by the dean/chair in the preliminary evaluation of courses taken to your enrollment form
- Get a copy of the Random Drug Testing Notofication Form
- Proceed to the Accounting Department for payment. - Claim your official Study load and Student Handbook. Failure to claim means no official enrollment
- Sign an undertaking for official accreditation of courses taken from other schools using Registrar form 22.
ETTEAP
- Go to the ETTEAP Office
- Fill-out Applicant Info Sheet, ETEEAP Pre-Application Form
- Submit requirements
- Proceed to the SAO and present pre-application form
- Go to the Accounting Department and pay P300 for Testing fee and P50 for the digital picture
- Proceed to the OAS
- Fill out Enrollment Application Form
- Present the receipt of payment for picture taking and get the schedule of Entrance Exam
- Take the test at the Testing Room/Guidance Office upon presentation of the Testing Permit.
- Get Enrollment Admission Slip
- Proceed to the ETO for the encoding of courses and printing of enrollment form upon presentation of ETEEAP pre application form and Enrollment Admission Slip
- Proceed to the ETEEAP Office for the signaturre of your enrolment form by the ETEEAP Coordinator
- Go the Accounting Department and pay P5 000 for the enrollment fee
- Go the the ETO and claim your ID Card upon submission of the remaingin copy of your enrollment form
Graduate Studies
- Go to the Graduate Program Office
- Fill out Enrollment Application Form
- Submit requirements
- Go to the ETO for the ussuance of Student ID Number
- Go to the Accounting Office and pay P250 for the testing fee and P50 for the digital picture
- Proceed to the OAS
- Submit requirements
- Present the receipt of payment for picture taking and get the schedule of Entrance Exam
- Take the Entrance Exam at the Testing Room/Guidance Office upon presentation of the Testing Permit to the Guidance Counselor and wait for the test result interpretation, interview and issuance of the duly signed enrollment admission slip
- Get Enrollment Admission Slip
- Proceed to the Graduate Program Office
- See the Dean/Chair for approval of Enrollment Admission
- Courses taken from other schools must be initially evaluated bu the Dean/Chair using the prospectus/checklist as basis of you tentative course load. Maximum of 12 units only shall be credited
- Present tentative course load to the enrollment adviser for encoding and printing/releasing of the duly signed enrollment form
- Attach the prospectus used by the dean/chair in the preliminary evaluation of courses taken to your enrollment form
- Proceed to the Accounting Office for payment. (Students who will pay in full are entitled to a cash discount which will be evaluated by th Accounting Department, payment must be made within 3 working days for your subjects to be officially enrolled.)
- Proceed to the ETO

- Claim temporary study load. Check immediately if the study contains all the subjects you intend to enroll and the corect schedule of classes. Changing (adding/dropping) of subjects is discouraged.
- Wait for ID card
- Go to the Registrar's Office not later than the date reflected in the temporary study load and clam official study; otherwise, you are not considered enrolled
SHS
- Go to the Office of Admissions and Scholarships (OAS)
- Fill-out Enrollment Application Form
- Submit requirements
- Get Student ID Number
- Proceed to the Accounting Department (disregard this step if with reservation).
- Pay P50 for digital picture, P150 for tresting fee, P800 for enrollment fee - optional and must use separate payment slip
- Proceed to the OAS (Disregard this step if with reservation)
- Present the receipt of payment and testing
- Wait for interview and rest result
- Proceed to the SHS office
- Present test result for the printing of you Enrollment Form
- Review and sign the enrollment form
- Proceed to the Accounting Department for payment. If with reservation - pay P500 for enrollment fee upon presenting the reservation slip. If without reservation - pay P800 for enrollment fee (Disregard this step if and enrolment fee has been made in step 6
- Proceed to the ETO
- Submit remaining copy of enrollment form
- Claim Official Study Load and ID card
