CROSS ENROLLEES REQUIREMENTS

Requirements

Student of other institution may wish to cross enroll in CLSU for subject(s) needed for his/her graduation.
  • Cross-enrollee must present a cross registration permit from the Registrar of his/her mother institution specifying the total number of units and the subject(s) to be cross enrolled.
  • 1.4.3 He/She must qualify in the physical or health examinationconducted by the University physician.
  • He/She must present a certificate of good moral character.
  • Admission is subject to the availability of slot for the subject(s) being cross-enrolled.
  • He/She must pledge to abide by and comply with all the rules and regulations of the University/College.
  • Student from other institution may be allowed to cross-enroll within the registration period only.

Undergraduate Programs Requirements

Requirements

Freshmen
  • Original copy of High School Card (Form 138), for Beginning Freshman OR Transfer Credentials/Honorable Dismissal, Transcript of Records/Certification of Grades, Certification of Good Moral Character from previous school, for Transferee
  • Original and Xerox copy of Birth Certificate, issued by the Philippine Statistics Agency (PSA)
  • Two pcs 2”x2” colored ID photo with name tag (white background). Computer-generated and cropped photos will not be accepted
  • Original copy of CLSU-CAT results
  • Filled-up Application for Enrollment (ACA.OAD.YYY.F.001)
  • Accomplished Student Directory Form (ACA.OAD.YYY.F.007)
  • Medical Certificate issued by CLSU Hospital (see Step 2)
  • Lodging Permit issued by Office of Student Affairs (OSA) (see Step 3)
  • Additional documents for recipients of Scholarship/Study Grant
           For Top 20 CLSU-CAT Qualifiers. The notice/letter sent to you by OAd.
For Children of CLSU Employee. Affidavit of your parent (CLSU employee). Secure form from the Office of the University President). For other scholarship/financial grantees. Certificate of scholarship/financial grant  

 

Enrollment Procedures

Requirements

STEP 1. APPLY FOR ADMISSION to the curricular program you wish to pursue. Get BLOCK ASSIGNMENT.
  • If a Beginning Freshman, you will be asked to present your high school card (Form 138).
  • If a Transferee (you have already enrolled in college/tertiary education in other school/university), you will be asked to present Transfer Credentials/Honorable Dismissal, Certification of Good Moral Character and Transcript of Records or Certification of Grades; failures, if any, must not be more than 25% in all units taken during the last semester. (NOTE: There are specific set of admission requirements for a particular curricular program/course. If you cannot be admitted in your “priority course”, you may wish to seek admission in other curricular program/course.)
STEP 2. UNDERGO A COMPLETE PHYSICAL-MEDICAL-DENTAL or HEALTH CHECK-UP at the CLSU Infirmary. You are expected to report for this check-up on the date indicated in your letter (Box 1, No. 7). Get the results. If you are issued a medical certificate reflecting your fitness to study in CLSU, you have satisfied another admission requirement.

STEP 3. GET LODGING PERMIT at the Office of Student Affairs (OSA) You will be asked to present your Medical Certificate. If you will be accommodated in the University dormitory, you will pay the dormitory fee that ranges from P900.00 to P2,500.00 per semester.

STEP 4. PROCESS YOUR ENROLLMENT
  • 4.1 Submit to OAd the required documents for admission. 
  • 4.2 You may visit www.oad-clsu.com/registration to print your official Registration Form (ACA.OAD.YYY.F.006). Web-based printing of Registration Form will be made available by first week of July. Schedule of your classes can be seen in your Registration Form.

Undergraduate Program Requirements and Procedure (Transferee)

Requirements

Applicants must meet all the prescribed admission requirements of the University and the concerned College.
  • Must qualify in the CLSU-CAT if at least 36 credit units of college courses have not yet been completed
  • Must have complete and valid credentials
a. Copy of birth certificate
b. Two 2”x2” colored ID pictures
c. Certification of good moral character
d. Certificate of honorable dismissal/ transfer credential
e. Copy of grades signed by the Registrar or official transcript of records
  • Must qualify in the physical and health examination conducted by the University physician
  • Must present a duly accomplished application form
  • Others, as prescribed by the concerned College
  • Applicants must not have more than 25% failure in all units taken during the last semester
  • Only students who have enrolled in any course leading to a degree program are admitted as transferees.
  • Transferees from SUC institutions shall be admitted and treated as regular students in accordance with the existing policies of the University/College.
  • Official Transcript of Records (OTR) shall be submitted prior to admission for the following semester: otherwise; enrollment will not be permitted.
  • Transferees must pledge to abide by and comply with all the rules and regulations of the University/College.
  • Admission of transferee is subject to availability of slot.
  • Admission of transferee is recommended by the concerned Department/College.
  • Transferees must comply with the provisions on accreditation/validation of courses.

Foreign Student Requirement and Procedure

Requirements

Applicants must meet all the prescribed admission requirements of the University/ College and the course applied for:
  • Must qualify in the University/ College Admission Test
  • Must have complete and valid credentials
a. Copy of birth certificate
b. Two 2”x2: colored ID pictures
c. Certificate of completion of a secondary curriculum
d. Original transcript of records
e. Personal history statement
f. Affidavit of support
g. Alien certificate of registration (ACR)
h. Student visa
i. Certificate of Proficiency in English issued by the CLSU Department of English and Humanities for a fee, for students who come from countries where English is not the medium of instruction
j. Security clearance from his/her embassy
  • Must pay a non-refundable application fee
  • Must qualify in the physical or health examination conducted by the University physician
  • Must present an approved application for admission
  • Others, as prescribed by the concerned College
  • Applicants must meet all the prescribed requirements by the DFA, the BID and the CHED.
  • A foreign student may be admitted based on availability of slot in the course applied for.
  • He/she must pledge to abide by and comply with all the rules and regulations of the University/College.

RETURNING STUDENTS

Requirements

  • A returning student is one who is not enrolled in the University in the preceding semester(s).
  • Admission of a returning student is dependent on his/her previous scholastic standing.
  • He/She must qualify in the physical/health examination administered by the University.
  • He/She must pledge to abide by and comply with all the rules and regulations of the University/College.
