  2. Proceed to the Program Head of the department you are applying to, for interview and referral for the entrance examination.
  3. Follow appropriate procedure for enrollment as above.

Additional Requirement for Filipinos Who Studied Abroad

  • Photocopy of Passport

Pre-Enrollment Procedure

NON RESIDENT ALIENS

  1. Submit requirements to SRMS for verification of records.
  2. Proceed to the Program Head of the department you are applying to, present English Proficiency Certification and undergo interview. If student is unable to present English Proficiency Certification, he/she shall take Intensive English Lessons Part A.
  3. Get referral from Program Head to the Guidance and Counseling Section for the entrance examination and to the English Department for the English Proficiency Test.
  4. Proceed to the Cash Section and pay the entrance and psychological examination fees. Those applying to the Doctor of Dentistry program, shall also pay the fee for the Qualifying Examination for Dentistry.
  5. Take entrance and psychological examinations (and Qualifying Examination for Dentistry, if applicable) on the specified dates.
  6. Proceed to the International Languages Department for English Proficiency Test and Interview.
  7. Claim Application for Admission Form from GCS. Claim also the English proficiency test result.
  8. Return to the Program Head test results for recommendation of acceptance and proceed to the Unit Registrar for approval, acceptance and enrollment.

Admission Requirements for Non-Resident Aliens

  4. Drug Test
  5. Photocopy of Passport
  6. Appropriate Visa
  7. Informed Consent Form

CONTINUING STUDENTS

Enrollment Procedure

  1. Proceed to the enrollment adviser and get Grades Clearance/Adviser’s Form and Routing Form. For graduating students, accomplish Information Sheet for Graduating Student Form and submit to the Program Head’s office.
  2. In the case of students with deficiencies, proceed to the SRMS for verification/clearance of deficiencies.
  3. Proceed to enrollment area for advising on subject/class schedule.
  4. Secure computer generated Certificate of Matriculation (COM) which includes assessment of fees.
  5. If entitled to a discount, present approved supporting document to the Accounting Section and request for re-assessment of fees.
  6. Proceed to the Cash Section or any authorized bank for payment of fees.

IMPORTANT: Slot reservation for a class is good for 24 hours only.
Note: If payment will be done at any authorized bank, the COM shall be stamped “PAID” at the Accounting Section before submission to the Student Records Management Section (SRMS).

  1. Proceed to the Health Services Section for medical, dental and/or other requirements of the Department.
  2. Proceed to the Uniform Section, present validated COM and get uniform.
  3. Proceed to the DOCUMENT FINAL SUBMISSION AREA to submit all enrollment documents including Grades Clearance/Adviser’s Form and Routing Form. Retain the student’s copy of the Certificate of Matriculation stamped “Enrolled”.
  4. For Foreign students: Submit all documents to the SRMS.

Post-Enrollment Procedure

  1. Proceed to the Library Section for the Barcode and present validated COM.
  2. Proceed to the Teaching and Learning Technology Section (TLTS) for ID and present validated COM.
  3. Return to the Program Head and have your COM stamped “Registered.”

Other Policies on Enrollment

  1. You are officially enrolled only when: Your Certificate of Matriculation (COM) has been machine validated at the Cash Section; and
  2. Your Certificate of Matriculation (COM) stamped “Enrolled” has been turned over to the Document Final Submission Area.
  3. A fine will be imposed for late registration.

Second Degree (Undergraduate)

Requirements

Pre-Enrollment Procedure

  1. Proceed to the Program Head for initial interview.
  2. Go to the Cash Section for payment of fee for drug test.
  3. Take drug test at the Clinical Laboratory within the day of payment.
  4. Claim drug test result at the Program Head’s Office on the designated date.
  5. Accomplish Application Form for Admission.
  6. Return to the Program Head’s Office to submit all requirements and for evaluation of professional subjects.
  7. Proceed to the Office of the Vice President/Dean of Studies for evaluation of General Education subjects/units.
  8. Return to the Program Head’s Office for submission of enrolment requirements, final interview and approval of enrolment. If admitted, sign Student’s Pledge.
  9. Follow enrollment procedure as New Students. Documents to be submitted to SRMS are: Application for Admission Form, transfer credentials, transcript of records, transferee’s and parent’s/guardian’s waiver, drug test results, pictures, birth certificate (NSO), character testimonies, curriculum checklist, Adviser’s Form, Routing Form Informed Consent Form and I-Card (for aliens).

  3. Good character testimony from the authorized official of former school
  4. Two (2) copies of latest ID photo (1½” x 1½”)

  7. Transferee’s and Parent’s/Guardian’s Waiver
  8. Certificate of Eligibility for applicants to the Dentistry Department
  9. PRC ID for applicants to the Pharm.D. Program
  10. Entrance evaluation of subjects taken to be done by the Dean/Academic Department Heads
  11. Informed Consent Form

Transferees (Undergraduate)

Requirements

Enrollment Procedure

Documents to be submitted to SRMS are: Application for Admission Form, letter seeking admission with parent’s/guardian’s signature, transfer credentials, scholastic records, transferee’s and parent’s/guardian’s waiver, drug test results, picture, birth certificate (NSO), character testimonies, curriculum checklist, Adviser’s Form, and Routing Form, Waiver/Informed Consent Form, and I-Card (for aliens).

Incoming First Year (Undergraduate)

Requirements

 Freshmen Pre-Enrollment Procedures

  1. Proceed to the Guidance and Counseling Section (GCS) for application for entrance examination.
  2. Submit accomplished Application for Admission Form and one (1) 1 ½” x 1 ½” ID photo to the GCS; get schedule of examination and room assignment.
  3. Proceed to the Cash Section and pay the entrance examination fee.
  4. Take entrance examination at the specified date and place of examination.
  5. While waiting for examination results, prepare additional requirements for admission:
  • Original Form 138 / H.S. Report Card
  • Good character testimony from the High School principal
  • NSO copy of birth certificate (original and photocopy)
  • Two (2) 1 ½” x 1 ½” ID photos
  • For honor graduates, certification of the principal for the honor received.
  1. Claim test result from GCS and the accomplished application form for admission.
  2. Proceed to the Program Head to present the requirements and for interview. If admitted, sign Student’s Pledge.

Enrollment Procedure

  1. Proceed to Department enrollment area and present admission requirements to the enrollment adviser.
  2. Get copy of the Routing Form, Adviser’s Form and Curriculum checklist.
  3. Fill out Adviser’s Form and have adviser indicate the subjects, section, and schedule.
  4. Wait for computer generated Certificate of Matriculation (COM) which contains Student Number and assessment of fees.
  5. If you are entitled to a discount, present approved supporting document to the Accounting Section and request for re-assessment of fees.
  6. Proceed to the Cash Section or any authorized bank for payment of fees.

Graduate Admission Procedures

Requirements

  1. An applicant for a Masterals degree should be a Bachelor degree holder with a weighted average of at least 2.00 or its equivalent. He/She should also be proficient in English and shall manifest evidence of creativity and research potential.
  2. An applicant for a doctoral degree should have a Masterals degree with a weighted average of at least 1.75 or its equivalent, have at least 2 years of teaching or administrative experience or its equivalent, be proficient in English and manifest evidence of ability to think clearly, logically and creatively.
  3. An applicant shall be required to submit an original transcript of records with Special Order (S.O.).Applicants from state colleges and universities shall be required to submit only the original transcript of records Transferees should present transfer credentials in addition to Transcript upon enrolment.
  4. New students and transferees shall be required to take the Graduate School qualifying exam. Only students who completed their Masters  degree in CEU shall be exempted from the qualifying exam when they enroll in the doctoral program.
  5. The Graduate School Associate Dean shall interview the applicant and shall refer him/her to the School Dean concerned who, in turn, shall evaluate his/her scholastic records.
  6. Transfer applicants may be given credits for a maximum of nine (9) units in the Masters program or twelve (12) units in the Doctoral program of subjects taken from a reputable school upon advice and recommendation of the School Dean concerned.
