  • GRADUATE SCHOOL PROGRAMS

    GRADUATE SCHOOL PROGRAMS

    • Complete information on the system/online: www.ceu.edu.ph/ceuadmission/form.
    • Go to Admissions Office located at LAH Bldg., Ground Flr. for printing & checking of Application for Admission Form and submission of (2pcs.) 1.5x1.5 ID picture.
    • Prepare the following requirements: Original Transcript of Records with S.O. number, Transfer Credential (If graduate from another school), for female photocopy of PSA Birth Certificate/Marriage Contract.
    • Go to the Graduate School Dean’s Office for initial interview and get the Endorsement Form for drug testing.
    • Proceed to the Clinical Laboratory for drug test within the day of payment.
    • Pay the Drug Test fee to Cash Dept./ Division/Section.
    • Claim drug test result at Graduate School Dean's Office on the designated date and get referral for Guidance and Counseling Department (GCD) located at SDV Bldg., 3rd Flr., Room 303 and get Endorsement Form for Graduate School Admission Tests(Mental Ability Test, English Proficiency Test, Personality Test).
    • Pay the Graduate School Admission Test to Cash Dept./ Division/ Section.
    • Proceed to GCD to take the Graduate School Admission Test.
    • Claim the result of examination at the Graduate School Dean’s Office on the designated date.
    • Registration for examination is at least 15 minutes before the chosen schedule.
    • Return to the Admissions Office to get the following:
      • Student Number
      • Routing Form for Enrollment
      • Informed Consent Form
      • Student’s Pledge
      • Waiver/Informed Consent Form (DPA)

    Procedures

    Freshmen

    • Complete information on the website link: www.ceu.edu.ph/ceuadmission/form.
    • Go to Admissions Office located at LAH Bldg., Ground Flr. for checking of Application Form and submission of (2pcs.) 1.5x 1.5 ID picture and grade 11 card / Form 138 with General Weighted Average
    • Pay the entrance examination fee at the Admissions Office.
    • Take entrance examination. Registration is done 15 minutes before the given time schedule.
    • Results of the exam will be sent thru email.
    • Prepare the following requirements for admission:
      • Grade 11 Form 138/Card with general weighted average
      • Certificate of Good Moral Character from the High School Principal
      • Original Form 138/Grade 12 Report Card/PEPT Result/ ALSA&E Result
      • PSA copy of Birth Certificate
    • Return to Admissions Office to sign the following requirements for admission.
      • Routing Form for enrollment
      • Student’s Pledge
      • Waiver or Informed Consent Form
      • Declaration of Health Status Condition

    First Year from CEU Senior High School
    • Complete information on the system/online (www.ceu.edu.ph/ceuadmission/form)
    • Go to Admissions Office located at LAH Bldg., Ground Flr. checking of Application Form and submission of (2pcs.) 1.5x1.5 ID picture
    • Prepare the following  requirements for admission:
      • Grade 11 Form 138/ Card with general weighted average
      • Certificate of Good Moral Character from High School Principal
      • Original Form 138/ Grade 12 Report Card/ PEPT Result/ ALSA&E Result
      • PSA copy of  Birth Certificate
    • Return to Admissions Office to get/sign the following requirements for admission or enrollment:
      • Routing Form for enrolment
      • Student’s Pledge
      • Waiver or Informed Consent Form
      • Declaration of Health Status Condition

    Resident aliens or Filipino studied abroad
    1. Complete information on the system/online (www.ceu.edu.ph/ceuadmission/form)
    2. Go to Admissions Office located at LAH Bldg., Ground Flr. for printing & checking of Application Form for Admission and submission of (2pcs.) 1.5x1.5 ID picture.
    3. Go to the Dean’s Office for initial interview and get the Endorsement Form for drug testing.
    4. Proceed to the Clinical Laboratory for drug test within the day of payment.
    5. Pay the Drug Test fee to Cash Dept or Division/Section.
    6. Claim drug test result at the Admissions Office on the designated date.
    7. Go to the Department Head’s Office for evaluation of core subjects and get Referral and Acknowledge form for Entrance Examination.
    8. Pay the Entrance Examination fee to Cash Dept/Division/Section.
    9. Go to the Guidance and Counseling Department (GCD) located at SDV Bldg., 3rd Flr., Room 303, presents referral from the dean/program head and receipt of examination fee and take the entrance examination.
    10. Registration for examination is at least 15 minutes before the chosen schedule.
    11. Go to Admissions Office within the day if the exam is taken between 8:00 A.M. to 3:00 P.M. and the following day if the exam is taken at 3:00 A.M.-5:00 P.M.
    12. Prepare the necessary requirements for admission:
    Filipinos Who Studied Abroad
    • Birth Certificate with Registry Number (Photocopy)
    • Certificate of Recognition
    • Original Transcript of Records
    • Photocopy of Passport
    Resident Aliens
    • Birth Certificate with Registry Number (Photocopy)
    • Certificate of Good Moral Character from High School Principal
    • I-Card
    • Original Transcript of Records
    • Recent 1.5x1.5 I.D. picture (2 pcs.)
    13. Go to the Office of the University Registrar (Window 7) for the checking of requirements
    14. Return to Admissions Office to get/sign the following requirements for admission/enrollment:
    • Routing Form for Enrollment
    • Student’s Pledge
    • Waiver/Informed Consent Form
    • Declaration of Health Status Condition

    Non-Resident Aliens
    • Complete information in the system/online (www.ceu.edu.ph/ceuadmission/form)
    • Go to the Admissions Office located at LAH Bldg., Ground Flr. for printing & checking of Application Form for Admission and submission of (2 pcs.) 1.5x1.5 ID picture.
    • Prepare the following for submission to the Office of the University Registrar (Window 7): appropriate visa, Certificate of Graduation, original Transcript of Records, photocopy of passport, police clearance (shall be authenticated by the Philippine Consulate abroad)
    • Go to the Dean’s Office for initial interview and get the Endorsement Form for drug testing.
    • Proceed to the Clinical Laboratory for drug test within the day of payment.
    • Pay the Drug Test fee to Cash Dept./ Division/Section.
    • Claim drug test result at the Admissions Office on the designated date.
    • Go to the Department Head’s Office for evaluation of core subjects and get Referral and Acknowledge form for Entrance Examination.
    • Pay the Psychological Test and Entrance Exam fees to Cash Dept./ Division/ Section.
    • Go to the Guidance and Counseling Department (GCD) located at SDV Bldg., 3rd Flr., Room 303 to take psychological test.
    • Proceed to International Languages Department for English Proficiency Exam.
    • Claim the result of examination at the Admissions Office on the designated date.
    • Registration for examination is at least 15 minutes before the chosen schedule.
    • Proceed to the Admissions Office for the following:
                       Examination results
                       Routing Form for Enrollment
                       Student’s Pledge
                       Waiver/Informed Consent Form
                       Declaration of Health Status Condition

    Transfer/Second Degree
    1. Complete information in the system/online (www.ceu.edu.ph/ceuadmission/form)
    2. Go to the Admissions Office located at LAH Bldg., Ground Flr. for printing and checking Application Form for Admission and submission of (1pc.) 1.5 x 1.5 ID picture.
    3. Go to the Dean’s Office for initial interview and get the Endorsement Form for drug testing.
    4. Proceed to the Clinical Laboratory for drug test within the day of payment.
    5. Pay the Drug Test fee to Cash Dept./ Division/Section.
    6. Claim drug test result at the Dean’s Office and gets the Referral and Acknowledgement Form.
    7. Go to the Guidance and Counseling Department (GCD) located at SDV Bldg., 3rd Flr., Room 303 and get endorsement form for Psychological Test.
    8. Pay the Psychological Test fee to Cash Dept./ Division/ Section.
    9. Take Psychological Test at the GCD.
    10. Proceed to the GCD for the interpretation of the result.
    11. Go to the Dean’s Office for further evaluation of result.
    12. Prepare the following additional requirements for admission:
    (Transfer Student)
    1. Certificate of Eligibility for Dentistry (for Dentistry applicants only)
    2. Certificate of Good Moral Character from Authorized Official of former school
    3. Original Scholastic Records bearing School Seal and Signature of the Registrar
    4. PSA copy of Birth Certificate
    5. Students and Parents/ Guardians letter seeking for admission
    6. Transfer Credentials
    (Second Degree)
    1. Certificate of Good Moral Character from Authorized Official of former school
    2. Entrance evaluation of subjects taken by the Dean/Program Head/ Academic Department Head
    3. Original Scholastic Records
    4. PSA copy of Birth Certificate
    5. Student’s and Parents/ Guardians letter seeking for admission
