SENIOR HIGH SCHOOL GRADUATES
- Original and Duplicate copy of Form 138 (Grade 12 Report Card)
- Original and Duplicate copy of Certification of Good Moral Character
- Original Columban College Classification Test (CCCT) Result
- Duplicate copy of Baptismal and PSA Birth Certificate
- Three (3) copies of recent picture (2x2)
- Duplicate copy of Marriage Certificate, if married
TRANSFEREES
- Original Copy of Certificate of Eligibility to Transfer (Transfer Credential)
- Original and Duplicate copy of Transcript of Records for Evaluation or a Copy of Scholastic records showing the subjects, number of units taken, and the grades obtained.
- Original and Duplicate copy of Certification of Good Moral Character
- Original Columban College Classification Test (CCCT) Result
- Duplicate copy of Baptismal and PSA Birth Certificate
- Three (3) copies of recent picture (2x2)
- Duplicate copy of Marriage Certificate, if married
PROCEDURES
- Go to columban.pinnacle.com.ph/columban/applicants
- Click “Apply” and fill out all the required fields.
- Check all the entries if correct.
- Click “Accept” and Print the Application Form.
- Affix your signature to the printed Application Form.
- Proceed to the Student Affairs Office for interview.
- Proceed to the Guidance and Testing Center for Registration and Classification Examination.
- Proceed to the respective Dean’s Office if the college chosen program is not aligned with the SHS track taken for the required bridging courses. (For freshmen only)
- Proceed to the Admissions and Registration Office for Approval and Submission of required credentials.
