Requirements
- Form 138 (High School Card)
- Original and Photocopy of Birth Certificate
- Three (3) copies of recent 2x2 picture
- Original and Photocopy of Baptismal Certificate
- College Scholastic Aptitude Test (CSAT) Result
- Good Moral Certificate
- Columban College Classification Test
- Marriage Certificate (for married applicants)
(Additional Requirements for Transferees)
- Transfer Credential (Honorable Dismissal)
- Transcript of Records (TOR)
Procedures
- Go to the Office of Student Affairs for your interview.
- Proceed to the Guidance and Testing Center for your Classification Examination and Registration.
- Go to the Admissions and Registration Office for the submission of requirements and approval.
