  • National Medical Admission Test (NMAT) Result (Original)
  • Notarized application for CEA with all the requirements therein including Application Fee payable to DMSFI Cashier’s Office (Note: Application Form is available at the DMSFI Registrar’s Office)
  • Medical Report issued by the school physician, which also include:
    • Laboratory examination (DMSFI General Hospital Laboratory):
      • CBC, blood typing, urinalysis, and fecalysis
      • Chest x-ray, PA view
      • Anti-HBs Antibody titer (for those with a history of Hepatitis B vaccination)
      • HBsAg and Anti-HBs Antibody titers (if without a history of Hepatitis B vaccination)
    • Neuro-psychiatric examination

Davao Medical School Foundation (DMSF) is a private higher educational institution in Bajada, Davao City. The school was founded in 1976 to address the need for more doctors and specialists in the rural areas and urban centers in Mindanao. The foundation is run by several institutions, namely Ateneo de Davao University, Brokenshire Memorial Hospital, the Development of Peoples’ Foundation, San Pedro Hospital, and San Pedro College. The school's affiliate hospital is the Davao Medical School Foundation Hospital (DMSF Hospital).

At present, DMSF offers bachelor's degree and doctorate degree courses in Medicine, Nursing, Dentistry, Biology, and Midwifery. Also available are graduate programs in Community Health, Participatory Development, and Health Professions Education.
