  • Medical Certificate (from the DHVTSU University Physician or other DOH accredited doctors)

Returning Students
 
  • Fully accomplished clearance from the cashier as of the last term enrolled.
  • Acceptance slip from the Dean of the receiving college or Director of the satellite campus.

Cross-Enrollees
 
  • Cross-enrolment permit from school or department currently enrolled in.
  • Certified true copy of evaluation form indicating the subject/s, course code and description of the subject the student wish to cross enroll.
  • Completely accomplished College Application Form
     
Shiftees
 
  • Certified True copy of grades
  • Guidance Counselor’s Recommendation
  • Endorsement from the Office of the Admission
  • Approved shifting form from the Dean of the receiving collge or director of the satellite campus

Note: Students with GWA lower than 85 and subject grades below 80 who wish to shift to another academic program with board exam shall not be allowed.

Don Honorio Ventura State University - Porac (DHVSU - Porac) is a state-run university campus part of the Don Honorio Ventura State University. Located in the highlands municipality of Porac, the campus was established in 2012 as part of the memorandum of agreement between the local government of Porac and the DHVSU. 

The Porac campus provides undergraduate degree programs in Elementary Education, Civil Engineering, Information Technology, Psychology, and Business Administration among others. Almost as identical with the other campuses in the system in terms of courses, the Porac campus, however, is the only campus in the system that offers an undergraduate degree in Social Work. And like the other campuses in the system, the Porac campus is officially recognized by CHED.
