    Fresshmen

    • Two pieces 2×2 colored ID Picture
    • Original and Photocopy of Transcript of Records or Certified True Copy of Grades
    • Authenticated copy of Course description (for crediting subjects)
    • Original Certificate of Good Moral Character
    • Honorable Dismissal / Transfer Credential Certificate
    • Original and photocopy of NSO Authenticated Birth Certificate
    • Transferee Recommendation Form

    Transferees
    • Two pieces 2×2 colored ID photos
    • Original and photocopy of NSO Authenticated Birth Certificate
    • Original Certificate of Good Moral Character
    • Honorable Dismissal / Transfer Credential Certificate
    • Original and photocopy of Summary of Scholastic Records
    • Course Description ( for crediting of subjects)
    • Transferee Recommendation Form

    Foreign Students
    • Two pieces 2×2 colored ID Picture
    • Current passport pages where personal information and other details appear (with recent arrival stamp)
    • Transcript of Records or Certified True Copy of Grades authenticated by the Philippine Foreign Service Post in his/her country of origin with English Translation
    • Police Clearance authenticated by the Philippine Foreign Post in his/her country of origin
    • Certified No Objection from the previous school (if applicable)
    • ICARD
    • Certificate of Good Moral Character
    • Authenticated Copy of Course Description (if local transferee)
    • Medical Certificate (if local transferee)


    Procedures
     
    1. PROCEED TO THE ADMISSIONS OFFICE. Present CAT Acceptance Letter and other required documents. Secure your Student Number and Student Information Sheet.
    2. PROCEED TO THE REGISTRAR’S OFFICE. Submit all documents for evaluation. Secure your Schedule and Assessment Form (SAF).
    3. PROCEED TO THE ACCOUNTING OFFICE. Have your fees assessed and pay at the Cashier. Be sure to get your Official Receipt  
    4. RETURN TO THE REGISTRAR’S OFFICE. Present your Official Receipt to claim your Certificate of Registration (COR). Have your photo taken for your Student Identification Card (ID). Return to the Admissions Office for final enrollment verification.
    5. PROCEED TO THE STUDENT AFFAIRS OFFICE. Undergo interview with the Guidance Counselor. Have your measurement taken for the uniform. Get your medical recommendation
