    Undergraduate Programs

    • Accomplished Application Form with one (1) passport size ID picture (white background)
    • Photocopy of 4th year grades or report card (back to back).
    • If PEPT (Philippine Educational Placement Test) qualifier: Original and photocopy of PEPT certificate of qualification for college.
    • Notice of acceptance
    • Original copy of report card.
    • Three (3) pieces passport size photo.
    • Certified true copy of birth certificate on NSO paper.
    • For those who stopped schooling: an original certification from the high school registrar clearly stating that the student has never enrolled in any college.
    • If PEPT qualifier, original PEPT certificate of qualification for college.

    Transferees
    • Accomplished Application Form.
    • One (1) passport size full color picture with white background.
    • Photocopy of certification of grades or Transcript of Records for evaluation.
    • Transfer Credentials: Recommendation Letter or Certification of Good Moral Character from the Guidance Counselor.
    • Notice of acceptance.
    • Official Transcript of Records or certification of grades (original copy).
    • Three (3) passport size, full color photos with white background.
    • Certified true copy of birth certificate on NSO paper.


    Procedures

    Undergraduate Programs
    1. Pay the Admission Processing Fee at the Accounting Office.
    2. Proceed to Admissions Office and present the official receipt of payment to secure your (1) Application Form; and (2) Test permit.
    3. The Admissions Office will give you the schedule of the Admission or Entrance Test. 
    4. The Admissions Office will advise you of the release date of entrance test results.
    5. Upon receiving your Notice of Acceptance, you may proceed to pay your reservation.

    Transferees
    1. Report to the Admissions Office for a preliminary interview and screening of documents.
    2. If qualified, please pay the Admissions Processing Fee at the Accounting Office.
    3. Present the official receipt of payment to the Admissions Office to secure application form.
    4. The Admissions Office will provide you your schedule for grade evaluation and interview with the Dean/Chair of the College where you wish to enrol.
    5. The College Dean/Chair will decide on the subject/s to be credited and that need/s validation and shall recommend you for admission.
    6.  If necessary, the College Dean/Chair may recommend that you take the Admissions Test. 
    7. Fill out the Request-for-Credit Form for approval by the Dean. The validating exam form is available at the accounting office. The form is presented for the Dean’s approval and for the assignment of test schedule. The Dean/Chair will submit the test result to the Registrar who will apply the proper grades credit. A validating exam fee must be paid before taking the exam. 
    8. Secure and submit the credited subject(s) form, duly signed and approved by the Dean at the Registrar’s Office.
