Requirements
- NSO Birth certificate
- Completed GSCS application form (may be acquired from the school office or download here)
- Certificate of Good Moral Character (except for nursery pupils)
- Form 138 (original report card)
- Form 137 (permanent record may be submitted later; only preschoolers are exempted from submission)
- 2 colored ID pictures (2×2)
- Entrance Examination fee
- Passing Rating for Entrance Examination
Procedures
Pre Enrollment (New Students)
- Accomplish Application form
- Submit accomplished application forms together with the requirements to the Registrar’s Office.
- Pay for Entrance Examination Fee, get schedule for exams, take the Entrance Examinations.
Enrollment Proper (Old & New Students)
- Get Registration Form from the Enrolling Teacher and accomplish same.
- Parents of transferees and new enrollees have to undergo orientation, date of which will be announced later.
- Submit duly accomplished Registration Form together with Form 138 (Report Card) to Assessment window for assessment of fees.
- Pay matriculation fees at the Cashier and see to it that the Admission Card is stamped PAID. Get Official Receipt.
- Submit Registration Card to SECTIONING for the child’s class assignation.
- Get the Admission Card for presentation to Class Adviser on School Opening Day.
