4. 2 Photocopies of Diploma

6. Fill up the application form and take the entrance exam at your selected campus.
Admission Requirements - Transferees:
1. Certification of Grades /TOR (Transcript of Records)
2. Certificate of Good Moral Character
3. Honorable Dismissal
4. 2 Photocopies of NSO Birth Certificate
5. 4 Pcs 2X2 Colored Pictures
Admission Requirements - Foreign Students:
1. Original Scholastic Record or Permanent Record
2. Certificate of Good Moral Character
3. 3 pcs. Photocopy of Passport (present orginal for authentication)
4. 3 pcs. Photocopy of Student Visa (present original for authentication)
5. 3 pcs. Photocopy of Police Clearance or NBI Clearance (present original for authentication)
6. 3 pcs. Photocopy of Quarantine Medical Clearance by the National Quarantine Office (present orginal for authentication)
