NEW STUDENTS:
A
new student is deemed regularly enrolled when and onlywhen he/she has satisfied the following requirements:
- Filled up Registration Form (available at the Admin Dept.)
- Submitted his/her original latest Report Card or Form 138 (Current RC)
- Submitted his/her original latest Report Card or Form 137 (OTR)
- Submitted his//her Birth Certificate
- Submitted his/her Baptismal Certificate
- Submitted his/her Certificate of Good Moral Character (if a transferee)
- Submitted his/her latest Medical Certificate
- Submitted 2 pieces 2×2 and 2 pieces 1×1 I.D Pictures white background with name tag
- Submitted a certified true copy of the Form 137 (if a transferee)
- Conditional Enrolment Form (For Transferees)
- Paid up the reservation fee
- Underwent required diagnostic/entrance exams with a fee
FOREIGN STUDENTS
- Form 137 (Official Transcript of Record) Translated into English
- Form 138 (Current Report Card) Translated into English
- Certificate of Good Moral Character
- Photocopy of Passport and Visa
- I-Card
- Medical Certificate
- Special Study Permit (Bureau of Immigration).
ENROLLMENT PROCEDURES:
Initial Assessment (Upon Inquiry).
- Payment of Entrance/Diagnostic Examination Fee
- Taking the Entrance Examination
- Personal Interview
- Submission of the Documentary Requirements to the Registrar.
- Filling-in the Registration Form.
- Payment of Reservation Fee or Payment of the Tuition or Miscellaneous Fees in any mode of payment chosen by the applicant.
- Picture Taking for the I.D. Card.
