Freshmen
Requirements
- Medical Results (Hepa-8 Test, Chest X-ray, CBC, Urinalysis, Fecalysis, lshihara Eye Test, Hearing Test)
- Entrance Exam Result
- Certificate of Good Moral Character
- 1pc.ID picture (2"x2")
- High School Card or Transcript of Records for transferees
- Psychological Exam
- NSTP (CWTS/ROTC)- choose between CWTS or ROTC
Procedures
- Payment at Cashier or Mlhuillier/PTA
- MIS (picture taking for school I.D.)
- Submit documents at Registrar
Graduate School
Requirements
- Certificate of Good Moral Character-original copy
- Certificate of Completion (Junior High School –original copy)
- Report Card – original copy
- 1 pc. 2x2 colored ID picture with white background
- Education Service Contracting Grantee Form (ESC- private schools)
- QVP Certification ( Qualified Voucher Applicants only-NON ESC)
- Checklist
- Personal Information Slip

- Submit requirements at SAS/Guidance
- Picture taking for school ID
- Down payment at Cashier
- Submit requirements at Registrar's office
- Signing of Registration Form – OFFICIAL ENROLLMENT
