Freshmen
- High school report card or Form 137
- Certificate of good moral character
- Photocopy of NSO birth and baptismal certificates
- Two (2) copies of recent, identical photos (2x2)
- ACR for foreign students
- Entrance Examination
Transferees
- Transfer credentials
- Certification of grades for evaluation from the school last attended.
- Certification of good Moral Character duly signed by a school official
- Entrance Examination
- Two (2) copies of recent, identical photos (2x2)
- Photocopy of NSO birth and baptismal certificates
Senior High School
- Original copy of report card or Form 138
- Certificate of Good moral character
- Photocopy of NSO birth and baptismal certificates
- Two (2) copies of recent, identical photos (2x2) with white background
- For Foreign Students: ACR and Grade Placement, Photocopy of Passport
- Entrance Examination
Transferees

Procedures
- Fill out application forms from the LCCM Admissions Office.
- Together with all your documentary requirements, submit them all to the Registrar’s Office
- Once approved you will have to pay for, take and pass the entrance examination on the schedule given
- Enrollment procedures shall then follow based on the academic calendar schedules at the start of the school year.
