  • Duly notarized letter of intent to study in National College of Science and Technology (NCST)
  • Six (6) original copies duly signed Student Personal History Statement (Form available at the Office of Student Affairs), containing his left and right thumb mark and 2×2 photo on plain white background taken not more than six months prior to submission
  • Scholastic Records authenticated by the Philippine Embassy/Consulate located in the student’s country of origin or legal residence
  • Photo copy of data page of passport showing photo, date and place of birth, date of issue and expiry authenticated by the Philippine Embassy/Consulate located in the student’s country of origin or legal residence
  • Birth Certificate or its equivalent authenticated by the Philippine Embassy/Consulate located in the student’s country of origin or legal residence
  • Notarized Affidavit of support to cover expenses for the student’s accommodation, subsistence, school dues and other incidental expenses
  • Authenticated Police Clearance from the country of origin. (For someone who has resided in the Philippines for more than 59 days, an NBI clearance is also required)
  • Quarantine Medical Examination by the National Quarantine
  • Two (2) letters of recommendation from the Principal, Adviser, Counselor or any official from school graduated

ALS Graduates/Passers
  • One (1) piece Photocopied ALS Diploma of the student.
  • Original ALS Examination Result of the student.
