  • Original Transcript of Records (Second Courser)
  • Two (2) photocopies of PSA Birth Certificate
  • Two (2) recent identical 2"x2" photos, white background
  • Clear photocopy of Marriage Certificate (for married female applicants)

Procedures

FRESHMEN

1.  Accomplish the Application Form and ;
  • Admissions Staff issues Student No. 
  • Pay the application processing fee of P220.00 at the Cashier’s Office
  • Secure the schedule for the FREE Scholarship Exam (optional).
2.  Submit the following requirements:
  • Original High School Card or F-138A
  • Original Certificate of Good Moral Character
  • Three (3) copies-recent identical 2”x2” studio photo, white background
  • Clear photocopy:  PSA birth certificate & HS diploma
  • Clear photocopy of Marriage Certificate (for married female applicants)
3.  Pay the medical exam fee: 

(a) P500 for all courses, except Maritime Education.  Proceed to Fatima University Medical Center for (1) X-ray - 1st floor  (2) Laboratory - 2nd flr.  Results will be forwarded to the school physician who will notify applicants with negative results only for further medication. 

(b) P900 for Maritime applicants.  Proceed immediately to the (1) Eye Clinic for color perception  (2) ENT Clinic for audiometry - 2nd flr - Fatima University Medical Center (FUMC).  Applicants wait for eye and ear examination results which must be submitted to the Admissions Officer; then to X-ray & Lab.

4.  Interview with Admissions Officer to include the signing of Statement of Commitment and Conformity; Promissory Note and Waiver re:  Documents (for incomplete requirements); Discuss also payment plan:  Full /cash or downpayment plus 2 or 3 installments.

5.  Payment of Fees:  Cash payment is accepted at the Accounting Office or designated Admissions Office Cashier and designated banks in Marulas, Valenzuela:  Unionbank, Metrobank, PNB, Eastwestbank, BPI, and Chinabank; Malinta: Landbank and Asia United Bank. For personal check payment,  proceed to the Student Accounting Office.   On the back of the check, please write:  (a)  Name of Student (b) Student No.  (c) College Program  (d) Home Address & Contact Numbers (landline and/or mobile phones)
 
ENROLLMENT:
6. Present Payment Receipt to Admissions Office Computer Encoder and claim Registration Card and Class Schedule upon submission of  all requirements (No. 2.)
 
7. Finally, proceed to the Admissions Office Validator. Claim the admissions kit, sketch of college uniform and redeem Credit Memo for scholarship discount, if any, immediately after enrollment or not later than June 30 or November 30 of the school semester.


TRANSFEREES/SECOND COURSES

1.  Accomplish the Application Form.  
  • Admissions Staff issues Student No. 
  • Pay the application processing fee of P220.00 at the Cashier’s Office. 
2.  For evaluation (credit subjects from previous school/s),  submit any one of the following documents:
  • Scholastic Records 
  • Certificate of Grades  
  • Class Cards  
  • Transcript of Records
 The evaluation sheet will be signed by both the Evaluator and Applicant.  Original copy remains in the student envelope; carbon copy to the applicant.
 
3.  Pay the medical exam fee: 

(a)  P500 for all courses, except Maritime Education. Proceed to the Fatima University Medical Center for (1) X-ray – 1st flr and (2) Laboratory – 2nd flr.  Results of both will be forwarded to the school physician who will notify only applicants with negative results for medication.

(b)  P900 for Maritime applicants.  Proceed immediately to Ophtha and ENT clinics – 2nd flr – Fatima University Medical Center.  Applicant waits for results of color perception test and audiometry which must be submitted to the Admissions Officer; then to X-ray – 1st flr & Lab – 2nd floor.

Our Lady of Fatima University Quezon City

Our Lady of Fatima University (OLFU) is a private university in the Philippines widely known for its paramedical courses. It was established in 1967 by Jose C. Olivares and his son-in-law, Dr. Vicente M. Santos. From the former Our Lady of Fatima Hospital, the founders transformed it into a training ground for healthcare providers paving way for the rise of Our Lady of Fatima College of Nursing. The University was recognized then as one of the top producers of passers in Nursing Board Examinations. OLFU continued its feat and expanded its facilities. It added other programs in its curriculum. Currently, the school has campuses in Quezon City, Valenzuela City, Antipolo, San Fernando (Pampanga) and Nueva Ecija. 

OLFU Quezon City's quiet atmosphere offers the same brand of topnotch education as in the other OLFU campuses. Its suburban location provides a gentle, homely atmosphere while being minutes away from metro Quezon City.  Established for the greater Quezon City Area, easy transportation is accessed as the nearby community is bustling with activity. A sub-campus in the Regalado area is also in place, that gives students access to the Fatima Eye and Rehabilitation Center, a Fatima University Medical Center establishment.
