Freshmen
- Photocopy of High School Card
- 2 pcs. 1x1 ID picture
- Official Receipt of Php200.00 (Examination Fee) from the Cashier's Office
- Photocopy of NCAE - National Career Assessment Exam
Transferees
- Photocopy of Transcript of Records, Honorable Dismissal / Certificate of Grades
- 2 pcs. 1x1 ID picture
- Official Receipt of Php200.00 (Examination Fee) from the Cashier's Office
Procedures
- Get Application Form from the Office of Student Affairs 3rd floor, Administration Building.
- Fill-out the Application Form.
- Pay the examination fee of Php200.00 at the Cashier's Office.
- Return to the Office of Student Affairs and submit the duly accomplished application form together with the admission requirements.
- Get your Examination Permit.
- Take the examination test on the date specified.
- Get your examination result.
