Grade Requirements
- General Weighted Ave. of 80% or higher during 4th year HS.
- For currently enrolled as 4th year: Xerox copy of at least 1st and 2nd grading period of grade in 4th year and Form 137 (Copy of Grades from 1st to 3rd year)
- For High School graduate: Form 138 (Report Card)
Other Requirements
- NSO Birth Certificate of Applicant
- Residence Certificate (Cedula) of either parent
- Proof of Residency, we shall be requiring any two (2) of the following documents of either parent:
- Voter's ID
- Company ID
- Driver's License
- TIN ID
- Passport
- Two (2) pieces 2x2 most recent ID picture (colored white background) with printed name at the back and signature.
- One hundred pesos (Php 100.00) as Examination Fee.
Note: The documentary Requirements must be submitted in a long brown envelope, the applicant's full name at the back.
Only Applicants with Complete Requirements (Original and Photocopy) shall be accommodated and will go through the Application procedure.
