New and Transfer Students
- Admission Slip (issued from the Student Development Center after passing the entrance examination and interview with the Department Heads)
- Recent Report Card with Learner’s Reference Number (LRN)
- Certificate of Good Moral Character
- 2 copies of Birth Certificate
- Three (3) copies of 1×1 ID picture with red background
- Education Service Contracting (ESC) certificate (for applicants who are ESC grantees from their previous school)
- Must pass the Entrance Examination
Foreign Students
- English translated school records
- 2 copies of English translated Birth Certificate
- Admission Slip (issued from the Student Development Center after passing the entrance examination and interview with the Department Heads)
- Special Study Permit (SSP)
- Three (3) copies of 1×1 ID picture with red background
- Photocopy of passport’s bio-page and latest stamp of arrival and authorized stay
- Must pass the Entrance Examination
