  • ADMISSION PROCEDURES

    Undergraduate Programs

    1. Applicants who graduated from other high schools shall proceed to the Cashier (Ground Floor) for payment of Entrance Examination
    2. For applicants who are high school graduates of St. Jude College, proceed to the Guidance Office for filling out of the Students Information Form and issuance of clearance for admission.
    3. Present the Official Receipt to the Guidance Office.
    4. Fill out the Student Information Form and take the Entrance Examination administered by the Guidance Office
    5. Secure result of examination from the Guidance Office

    NOTES:
    • For Nursing applicants, proceed to the Registrar's Office and present the examination result with the admission requirements
    • For other courses, proceed to the Dean's Office for interview and evaluation and proceed to the Registrar's Office
    • Registrar's Office validates the examination results and evaluates the admission requirements prior to issuance of Student Number
    • Applicant proceeds to the IS&T Office for assessment of tuition fees
    • Applicant proceeds to the Accounting Office for payment of tuition fees and issuance of ID
    • Applicant proceeds to the ID Section and presents COM for issuance of ID
    • Applicant proceeds to the Uniform Section (outsource) and presents COM for issuance of uniform

    Enrollment Procedures
    1. REGISTRAR'S OFFICE
    Registrar issues admission requirements/transfer credentials
    Advises student to proceed to Accounting Office for payment of Entrance Examination.
    1. ACCOUNTING OFFICE
    Accounting clerk accepts payment and issues an Official Receipt
    Proceed to Guidance Office and present OR for the Entrance Examination
    1. GUIDANCE OFFICE
    Guidance Office Personnel documents Receipt of Payment
    Administers the Entrance Examination
    Entrance Examination checked and issues result to student
    1. REGISTRAR'S OFFICE
    Validates result of entrance examination
    Checks Admission Requirements / Transfer Credentials from the Student
    Issues a Student Number
    Advises student to proceed to his/her respective college for entry level determination
    1. RESPECTIVE COLLEGE
    Validates result of examination
    Determines entry level of student based on Transfer Credentials
    Nursing student with an entry level of I or II - He/she is evaluated for subjects to be taken at the College of LAEd and referred to the Assessment Clerk/s at the gymnasium. (Skip Steps 6 & 7 and proceed to steps 8 & 9). If the student belongs to a college other than LAEd and Nursing, he/she proceeds to the respective college for dean's interview, evaluation, and assessment
    Nursing student with an entry level of III or IV - The student is advised to take the Qualifying Examination and proceed to the Accounting Office for payment. If the student belongs to a college other than LAEd and Nursing, he/she proceeds to the respective college for dean's interview, evaluation, and assessment
    1. IS&T OFFICE
    Assessment Clerk issues Assessment Form
    Informs student of its validity for forty-eight (48) hours or two (2) days after which re-evaluation and re-assessment have to be done. Advises student to proceed to the Accounting Office for payment of matriculation
    1. ACCOUNTING OFFICE
    Accepts payment and issues Certificate of Matriculation

    • Certificate of Good Moral Character
    • Photocopy of Birth Certificate
    • Four (4) 2x2 ID pictures
    • Four (4) long brown envelopes
    • Four (4) Business Envelopes
    • Four (4) mailing stamps
    • Entrance Examination Passing Results

    Transferees
    • Transfer Credentials such as Certificate of Grades/Class Cards and Honorable Dismissal
    • Official Transcript of Records
    • Requirements for Freshmen (New) from 2-8
    • High School and Elementary Report Cards
    • Four (4) 2x2 ID pictures
    • Four (4) Business Envelopes
    • Four (4) mailing stamps

    • Certificate of Good moral character
    • Report card
    • Certificate of JHS completion
    • QVR/ESC Cerificate
    • 3pcs long brown envelope
    • NSO certificate
    • 2 long mailing envelope

    • NCAE reuslt
    • 4pcs 2x2 colored ID
    • 4pcs 1x1 ID picture

    Junior High School
    • Two (2) recent 2 x 2 pictures (White Background)
