- Grade 10 Report Card with Learner Reference number (LRN) if Public Junior High School Student (Original)
- Certificate of Junior High School Completion
- Certificate of Good Moral Character
- ESC Certification Letter (For Private School G10)
- QVR Certificate (Qualified Voucher Applicants Only)
- 1 pc. 2×2 ID picture
- 4 pcs. 1×1 ID picture
Procedures
- Submit the accomplished application form. (Note: The accomplished application forms shall be submitted to the Marketing Office, along with two (2) copies of your most recent ID pictures, from Mondays to Fridays at 8:00AM to 6:30PM and Saturdays at 8:00AM to 12:00PM.)
- For Senior High School Applicants, no entrance exam for those who want to take ABM, STEM, and GAS. As for Tech-Voc - Maritime Specialization Applicants, they need to take the Ishihara Test (Colorblind Examination).
- Submit the requirements for enrollment.
- Pay the necessary tuition and miscellaneous fees at the Cashier’s window. (Note: The Registration office shall issue your Registration Card and Student number. You will also be scheduled for an orientation.)
UNDERGRADUATE/BACHELOR’S DEGREE PROGRAMS

- Form 138 (Report Card)

Transferees
- Honorable Dismissal/ Transfer Credentials
- Certificate of Good Moral Character
- 4 pcs. 2×2 ID picture
- 4 pcs. 1×1 ID picture
- Birth Certificate (1 original, 1 photocopy)
Procedures
- Submit the accomplished application form. (Notes: The accomplished application forms shall be submitted to the Marketing Office, along with two (2) copies of your most recent ID pictures, from Mondays to Fridays at 8:00AM to 6:30PM and Saturdays at 8:00AM to 12:00PM. Upon submission, you will be given the College Admissions Test Permit.)
- Accomplish the form and pay the testing fee at the Cashier.
- Present the accomplished College Admissions Test Permit.
- Take the College Admissions Test. (Note: The College Admissions Test includes the Aptitude test, the Psychological Exam, and the Ishihara Test.)
- Submit the requirements for enrollment.
- Pay the necessary tuition and miscellaneous fees at the Cashier’s window. (Note: The Registration office shall issue your Registration Card and Student number. You will also be scheduled for an orientation.)
