  • Report Card with GWA or its equivalent (Form 138) (Photocopy)
  • Good Moral Certificate (Photocopy)
  • NSO/PSA Certified Birth Certificate (Photocopy)
  • Mayor's Endorsement (Photocopy)
  • Proof of Residency/Voter's ID (Photocopy)
  • Medical Certificate (Photocopy)
  • 2 pcs ID pictures (2x2 white background)
  • 1 pc. long brown envelope

Procedures
  1. Fill up admission form.
  2. Submit all necessary documents directly to the assigned Guidance and/or Admission Officer
  3. Pay the admission fee of Php100.00 only to the Cashier's Office.
  4. Secure admission test at the assigned schedule.
  5. Submit to an interview to be administered by the Program Head or by the College Dean.
  6. See the list of the qualifiers which will be posted at the College bulletin boards.
  7. For admitted students, sign the Behavior Contract and secure enrollment permit/ follow enrollment procedures.
