    • Must pass the Interview and College Entrance Exam
    • General Weighted Average (GWA) of at least 80% in Fourth Year High School
    • Accomplished application form
    • Personal interview with the Admissions Officer
    • 500 php (Entrance Exam Fee)
    • Transcript of Records or Copy of Grades  (For Transferees)
    • Bring 5 copies of 2x2 pictures

    1. Call the Admissions Office to indicate your preferred date for interview and for taking the College Entrance Examination.
    (Note: Walk-in Applicants will be given application form and will choose their schedule for the interview and the examination.)

    Interview and Exam Schedule
    Monday to Friday (8:00am or 1:00pm)
    1. Once the schedule has been set, the applicant must come to the Admissions Office (Rm 119) to submit the Application Form.
    2. Once done, the Admissions Officer will conduct an interview. After the interview, the applicant will be given Exam Permit.
    3. Present entrance exam permit and payment for the Entrance Exam Fee at the Accounting Office (Cashier's Window).
    4. Show exam permit together with the receipt to the psychometrician in-charge in the Guidance Center.
    5. Take the Entrance Examination.
    6. After the examination, wait for the result. (Note: Within 3 working days, the applicant will be informed through phone call or text. If passed, the applicant will be informed about the schedule of medical tests.)
    7. Get the Doctor’s request slip for the said tests and the schedule of Medical and Dental Examination at the Admissions Office.
    8. Next, Proceed to the Accounting Office (Cashier’s window) and pay 315 php for the lab fee.
    9. Present the receipt to the Medical Technologist in-charge at the College Medical Science Skills Lab (Rm.308) together with the required specimen. (Note: Laboratory Tests include Urinalysis, Fecalysis and CBC.)
    10. Once done, proceed to the SJDEFI Hospital X-ray Department and present the Doctor’s Request Slip for the X-ray test
    11. Get Charge Slip for payment (amounting to 390 php) for X-ray procedure at the Hospital Accounting Department.
    12. Go back to the X-ray Department and Present the Receipt to undergo the X-ray procedure.
    13. Get the Lab result at the College of Medical Technology office (Rm.109) and X-ray result at the X-ray Department SJDEFI Hospital.
    14. Present the results to the College Nurse at the College Clinic (Rm.104) to be accommodated for medical and dental examination.(Note: If cleared for enrolment, Medical and Dental clearance will be issued to the student.)
    15. Present Medical and Dental Clearance at the Admissions Office (Rm. 119) to get Notice of Temporary Admission.
    16. Show all requirements for enrolment on the scheduled dates at the Admissions Office for checking purposes. (Note: If the documents are complete, the Admissions Officer will endorse the applicant to the Registrar’s Office (Rm.122) for enrolment.)
