Freshmen
- Senior High School Report Card (Form 138)
- Birth Certificate
- Marriage Certificate (for married applicants)
- Certificate of Good Moral Character
- 4 copies of 2x2 picture
- 4 pieces long folder
- 2 pieces long brown envelope
Transferees
- Honorable Dismissal/Transfer Credentials
- Birth Certificate
- Marriage Certificate (for married applicants)
- Certificate of Good Moral Character
- 4 copies of 2x2 picture
- 4 pieces long folder
- 2 pieces long brown envelope
Procedures
- Attend the initial interview with the Program Head.
- Get a copy of the testing fee slip.
- Pay admission test fee at the Cashier’s Office and get the signed entrance examination permit.
- Go to the Registrar’s Office to accomplish the Application for Enrollment form.
- Proceed to the Guidance and Testing Center for the scheduling and administration of entrance examination.
- Go to your respective Program Head to get the result of approval/disapproval to enroll for the issuance of plotting form.
- Present the accomplished plotting form along with the other requirements to the Registrar’s office.
- Go to the Cashier’s Office for payment of fees.
- Proceed to the Coordinator of Student Affairs Office to secure a Student Permanent Record and ID application forms. (Note: Make sure that there are “ID Apply” stamps indicated on the registration form.
- Go to the Registrar’s Office to secure your class cards.
- Report on the first day of classes.
