                          FOR NEW STUDENTS

                          • Submit Documents
                          • Secure “Subject to be Enrolled Form (S.E.F)”

                          Step 2: Secure “Order of Payment Form”
                          (2nd Flr. – Circular Bldg.)

                          Step 3: Area 2 – (For Payment)

                          Step 4: Submission of photocopy of receipt
                          (2nd Flr. – Circular Bldg.)

                          Step 5: Submission of photocopy of receipt
                          (Registrar’s Office)

                          Validation of subject
                          (Ms. Anaflor Delos Santos)

                          Step 6: Encoding of Subjects

                          Step 7: Validation



Requirements for Graduate Studies:

• Entrance Exam Passer
       
1. Transcript of Record
                                      
2. Honorable Dismissal
                                        
3. Birth Certificate NSO Original
                                        
4. Marriage Certificate NSO Original
                                        
5. Barangay Clearance

• Medical and Dental Exam can be done at any Barangay Health Center
• CBC (blood count)
• Urinalysis (urine)
• Chest X-ray
• Dental
• Fecalysis (stool)
