  • SENIOR HIGH SCHOOL PROGRAMS

    • Entrance Results
    • Entrance Application Form (EAF) – Senior High School (SHS)
    • Photocopy of Report Card or Form 138 (with grades from the latest grading period)
    • Two (2) pieces 2 by 2 inches ID Picture
    • Philippine Statistics Authority (PSA) formerly NSO Birth Certificate
    • Admission Fee
    • Medical Certificate issued by the University Physician
    • National Career Assessment Examination (NCAE) Results
    • Other requirements to be submitted upon enrollment


     

  • UNDERGRADUATE/BACHELOR'S DEGREE PROGRAMS

    Requirements

    Freshmen

    • Original copy of Form 138 or High School Report Card
    • Certificate of Good Moral Character from the Principal
    • A clear photocopy of Certificate of Live Birth printed on National Statistics Office (NSO) Security Paper (please bring original copy for verification purposes only)
    • Two (2) pieces 2x2 colored picture
    • Result of Entrance Examination
    • Entrance Application Form (EAF)- College of  Law and Graduate School

    Transferees
    • Certificate of Transfer Credentials (Honorable Dismissal) from the school last attended
    • Transcript of Records
    • Certificate of Good Moral Character from the Dean
    • A clear photocopy of Certificate of Live Birth printed on National Statistics Office (NSO) Security Paper (please bring original copy for verification purposes only)
    • Two (2) pieces 2x2 colored picture

    Procedures
    1. Fill-out Enrolment Information Form (EIF)
    2. Present EIF for evaluation and approval to the Dean  (college) or his/her designated staff or Academic Coordinator (SHS) or his/her designated staff
    3. Pay tuition and other fees (down payment or full payment and departmental  fee) to the cashier
    4. Present approved EIF to Student Affairs and Welfare Office (SAWO) staff for school ID application
    5. Present approved EIF to ICT Office staff for e-mail creation
    6. Present receipt of payment and submits approved EIF to Academic Records Assistant; claims Registration Form and ID card from ARA

    Summer Term: March 2019 - April 2019


    Placement Test Start Date

    First Semester: January 2019 - March 2019
    Second Semester: April 2019 - May 2019

    Summer Term: April 2019
