Freshmen
  • USMCEE result (original)
  • Form 138/High School Report Card plus one (1) photocopy
  • One (1) original copy clear NSO Birth Certificate plus two (2) Photocopies
  • One (1) original copy of Certificate of Good Moral Character plus two (2) Photocopies
  • Certification of the Number of Graduating Students for Valedictorian and Salutatorian (at least 30 graduating students)
  • For scholars: Complete documents from sponsoring agency
  • Two (2) pcs of 2 X 2 ID picture (for USM-Main)
  • Three (3) pcs 1 X 1 ID picture( for USM-Main)
  • 2 copies 2x2 ID picture with name tag (for USM-KCC Campus)
  • If Student is enrolling in any of the courses enumerated below, a medical certificate must be secured from the University Hospital before seeking for Admission.
    • BS in Hotel & Restaurant Management (HRM)
    • BS in Food Technology (FT)
    • BS in Nutrition and Dietetics (ND)
    • BS in Nursing (Medical Certificate for this course issued by the USM Hospital is valid only within one (1) week).

Transferees
  • USMCEE result (original)
  • Transcript of Records from Previous School
  • Honorable Dismissal/Certificate of Transfer Credentials (Original)
  • One original clear copy NSO birth certificate plus two (2) photocopies
  • Certification of good moral character plus two (2) photocopies
  • If scholar: Complete documents from sponsoring agency
  • Two pieces 2X2 ID picture

PROCEDURE
1. Present necessary documents on your respective Guidance coordinator (depends on your college) and have your interview.
2. Accomplish your Trial Program form accordingly. This includes the writing of Schedule codes and securing of signature from an assigned academic adviser.
3. Register your Trial Program form on the Registration and wait for the hardcopy of assessment form.
4. Pay the tuition fees.
5. Pass all your enrollment documents to the final screening officer.

University of Southern Mindanao (USM) is a state university established in 1954 by Bai Hadja Fatima Matabay Plang as the Mindanao Institute of Technology in Kabacan, Cotabato. USM earned its university status on March 13, 1978 and currently has satellite campuses in Kidapawan City and Buluan, Maguindanao.
