Three (3) official copies of transcript of Records/ Scholastic Records, one (1) original copy and two (2) photocopies authenticated by the Philippine Embassy/ Consulate in the applicant's country of origin or legal residence. Official copies bear the original signature and the embossed/stamped seal of the issuing educational institution. The certification on the photocopies must be in original inked signature and seal.
Five (5) original copies of Personal History Statement (PHS) duly accomplished and signed by the applicant accompanied by original left and right hand prints. Each copy provided with 2 x 2 original latest photograph. PHS form may be obtained from the Philippine Embassy/Consulate in the applicants' country of origin.
Three (3) copies of affidavit of support and proof of adequate financial support duly notarized executed by your financial sponsor to cover expenses for student's accommodation and subsistence, school dues and other incidental expenses.
Three (3) photocopies of passport pages where name, photo, birth date and birthplace appear.
