- Four (4) pieces recent ID picture for the Library, Guidance, OSA/SSG, Registrar
Old Students:
- Payments of SSG Membership and other Fees at Office of Stucent Affiars (OSA)
- School ID
- Completion of Deficiences
Transferees:
- Honorable Dismissal
- Certificate of Good Moral Conduct
- One (1) piece Brown Envelope (long)
- Photocopy of Birth Certificate
- Four (4) pieces recent ID picture for the Library, Guidance, OSA/SSG, Registrar
