Senior High School
- PSA Certified Birth Certificate
- Certificate of Good Moral Character
- Photocopy of Grade 10 Report Card with LRN / E-SID
- Pictures 4 copies colored 1x1
- Certificate of JHS completion
- ESC Certification Letter from JHS (for ESC grantees)
- QVR Certificate (for Private school JHS completers)
Freshmen
- Form 138 (High School Card) or Form 137
- Certificate of Good Moral Character
- Accomplished Registration Form
- Pictures 4 copies colored 1x1
Transferees
- Transfer Credential (Honorable Dismissal)
- Certified True Copy of Transcript of Records or Certificate of Grades
- Passed the Entrance Exam and Interview
- Accomplished Registration Form
- Pictures 4 copies colored 1x1
Procedures
New students
- Submit requirements
- Scholarship Examination (Optional)
- Interview
- Payment
- Enrollment
- Issuance and filling out of Registration Form upon submission of Validated Deposit Slip from the bank
- Selection of Class Schedule and Section
- Issuance of Official Receipt, Student Copy of Registration Form, Request for Form 138/TOR and Freshmen Kit (subject to availability).
