- Permanent Record (Form 137)
- Report Card (Form 138)
- Original and Photocopies of Birth and Baptismal Certificates
- Accomplished Information Sheet and Application Form
- AAS Entrance Test and Interview
- Drug Test (For Grade 7 and above)
Requirements for Foreign Students
- Photocopy of Alien Copy of Registration (ACR)
- Photocopy of Passport
- Study Permit Issued by the Department of Education (DepEd)
- 2pcs. of Passport-size picture
Procedures
- Submit Admission Requirements along with your Certificate of Good Moral Character to the Registrar’s Office.
- Accomplish application forms and interview sheet given by the Registrar’s Office.
- Take the Entrance Examination (Note: Students who are unable to take the test on the specified date will be assigned a later schedule.
- Wait for the examination result (Note: Results will be released following the schedule given by the Examiner)
Reminders for Examinees
- Please bring two (2) sharpened pencils, exam permit and snacks on the scheduled examination day.
- Applicants should be at the lobby ten (10) minutes before the examination starts.
