For Freshmen:
- Original copy of High School Report Card/Original Copy of Form 138-A;
- Original copy of Result of System Admission and Scholarship Examination (SASE);
- Certificate of Good Moral Character issued by the Principal or Guidance Counselor;
- Birth Certificate, (original to be returned) and authenticated photocopy;
- Four (4) Copies 2x2 recent photograph; and
- The result of medical examination.
For Transferees:
- Honorable Dismissal (HD) and an evaluative copy of Transcript of Records;
- Original copy or result of System Admission and Scholarship Examination (SASE);
- Certificate of Good Moral Character issued by School head/representative;
- Birth Certificate (original to be returned) and authenticated photocopy;
- Four (4) Copies 2x2 recent photograph; and
- Result of medical examination.
- For Second Degree Applicants:
- Honorable Dismissal (HD) and an evaluative copy of Transcript of Records;
- Certificate of Good Moral Character;
- Birth Certificate (original copy to be returned) and authenticated photocopy;
- Four (4) Copies 2x2 recent photograph; and
- Result of medical examination.
For Foreign Students:
Foreign students may be admitted provided they meet the requirements of the Commission on Higher Education (CHED);
In addition, foreign students from countries where English is not the medium of instruction and/or nor the native language must submit a certification of English proficiency; and
The Office of the Registrar must have received all documents and other admission requirements at least one month before the start of registration.
Procedures:
At the beginning of every academic year registration procedures and enrollment flow are prepared and distributed in every college to serve as a guide for students;
Every student must be officially registered in order for him to earn credit for coursework;
No student shall be registered after 12% of the regular class meeting have been held; and
Late registration shall only be allowed upon certification of the concerned Dean and payment of a fine of P50.00.
For Cross-Registration
No student shall be registered in any college department or school of the University Campus without the permission of the Dean in which the student is primarily enrolled. The total number of the unit load, for which the student may enroll in two or more colleges, department or school shall not exceed the maximum load allowed by the University.
The student from another institution shall only be allowed upon written permission from the Dean, Registrar or head of Institution, stating the subjects and number of units for which the student is authorized to take in the University Campus.
The MSU System shall not give credit for any course taken by its students in any other institutions unless the taking of such course was authorized by the Vice Chancellor for Academic Affairs upon the recommendation of the respective Dean. The prescribed forms for permit to enroll must be duly accomplished and recorded by the Registrar.
