 NEW STUDENTS
1. Submit all requirements to the registrar
a. Submit all requirements to the registrar
b.Original and photocopy of Baptismal Certificate
c.Photocopy of the Report Card: Form 138
d.Fully accomplished recommendation form (The form may be obtained from the Guidance Center.)
e.Certification of good moral character signed by the previous Principal.
The Guidance Center informs the applicant about the status of his/her application. If the applicant passed, he/she is given a Permit to Enroll slip.

TRANSFEREES * (excluding Fourth year and Grade Six entrants)
A. All transferees should :
1.Report Card
2.A photocopy of the Birth and Baptismal Certificates
3.Alien Certificate of Registration (ACR)
4. Certificate of Eligibility for admission from the DepEd, if necessary

FOREIGN STUDENTS
A. Present the following requirements to the Registrar's Office at least, three days prior to the date of testing.

1. Have a general average grade of at least 80% and no failing mark in the final grade of any subject.
2. Pass the entrance tests and interview.
3.Be a regular student upon enrolment at St. Mary's Academy
