  • Certificate of Good Moral/ No disciplinary record
  • Copy of Report Card and Grades from the previous school year; No failing grade
  • Certificate of honors obtained issued by the Principal's Office(optional)
  • Certificate of Indigency(optional)
  • Needs-based discount essay application
  • Entrance exam score should not be lower by the determined passing grade by the Committee.
