  • Two (2) recent and identical 1" x 1"; pictures signed at the back by the applicant.
  • Official Receipt of Application Fee (P 20.00) and CAT Fee (P 125.00) to be paid by the applicant to the University Collecting Officer. All fees are non-refundable.
  • One (1) Long Folder (white)

Capiz State University (CAPSU) is a public academic institution located in the province of Capiz. The school came into existence after merging two educational institutions, the Mambusao Agricultural and Technical College and the Capiz Agricultural and Fishery School,in 1980. The school was then known as the Panay State Polytechnic College (PSPC). Eventually, the institution got its university status in 2004 and was renamed into its present university name. CAPSU was able to expand over the years of its operations, establishing many campuses all over the province: Roxas City (Main), Dayao, Pontevedra, Pilar, Burias, Dumarao, Mambusao, Tapaz, Sigma, and Sapian. 

CAPSU specializes in providing education and training in the fields of Agriculture, Fishery, Science, Technology, Arts and Humanities, and Education. It offers a wide array of undergraduate and graduate programs, ranging from Entrepreneurship to Industrial Technology. Also available is a senior high school program featuring Academic and Technical-Vocational-Livelihood (TVL) strands. The university also has several extension programs which aims to help its local in terms of production and livelihood. Some of these extensions are Mobile Technology School (MTS), Serbisyo sa Komunidad Aton Ihatag (SKAI), Mobile Business Administration, Monitoring and Evaluation (MOV), and Community Disaster Awareness and Preparedness (CDAP).
